Why does every copy of this release that I've seen for sale have a pencil-sized hole punch through the jacket, generally near the "o" in "Coil" ?

Those punch holes usually mean you're dealing with a so-called cut-out. As far as I know, these were unsold copies that were returned to the distributor, who would then "cut out" the release by drilling a hole or sawing a corner off the cover. It also meant that the artist would not be paid over those specific copies. Cut outs like this would then be resold in big numbers, with a discount, to music stores. Those stores would have 'cut out bins' or sell 'em as clearance records.
